Self-Ligating Brackets

Self-ligating brackets are essentially developed to accelerate and optimize tooth movement while providing greater comfort and reducing the frequency of patient visits to the orthodontist. These brackets offer significant advantages over traditional brackets by providing advanced and innovative treatment with smaller dimensions and fewer features.

One of the most important features of self-ligating brackets is their unique design, which allows them to close automatically without the need for elastic or metal ties. This type of bracket uses a special sliding mechanism to hold the archwires in place, reducing friction and increasing the speed and efficiency of tooth movement. Its special design also provides an easier and more comfortable treatment for patients by reducing the amount of force on the teeth.

Other advantages of self-ligating brackets compared to traditional brackets include:

  • Fewer Visits: Patients will experience fewer visits to the orthodontist.
  • Efficient Sliding Mechanism: The unique sliding door design of these brackets makes opening and closing easy, thus shortening the duration of orthodontic sessions and improving patient comfort.
  • Faster Tooth Movement: Faster initial tooth alignment.
  • Reduced Friction: Lower friction between the archwire and bracket in the early stages of treatment.
  • Hygiene: Self-ligating brackets are more hygienic as they do not require elastics, which reduces bacterial buildup and contamination.
  • Reduced Risk of Root Resorption: Lower risk of root resorption in patients with short roots.
  • Less Pain: Using flexible Copper-NiTi archwires can create forces below 100 grams, providing greater comfort.
  • Increased Patient Comfort: Enhanced comfort during treatment and archwire changes.

Disadvantages of Self-Ligating Brackets

  • Torque Control: Traditional brackets offer better torque control compared to self-ligating brackets, though the difference between active and passive self-ligating brackets is minimal.
  • Bracket Thickness: Self-ligating brackets are slightly thicker to accommodate the clip, but newer models have become thinner and are nearly undetectable by patients.

Why is Pactive® Considered the Best Self-Ligating Bracket in the Market?

The American company IOS produces Pactive, a high-quality self-ligating bracket with several unique features:

  • Versatile Function: As previously described, the Pactive® bracket can function as both an active and passive bracket depending on the wire size used. A 16x22 wire size acts as the transitional size from passive to active. Using a 16ss wire, the system functions as a pure passive system for sliding mechanics and space closure.
  • High Weld Strength: The Pactive bracket has a substantial amount of welding connecting the body to the base, ensuring the bracket will not detach from its base.
  • Reduced Space for Debris: Compared to other self-ligating brackets (SLB), the Pactive bracket provides more space between the clip and the base, reducing the possibility of debris buildup in the bracket tunnel. This minimizes the risk of clip getting stuck, making wire changes easier.
  • Beveled Bracket Entrance: Studies show that increasing the bevel angle of the bracket reduces frictional forces. IOS has incorporated this important concept into the design of the Pactive bracket.
  • NiTi Smart Clip: The smart clip is made from NiTi to maintain its shape. Additionally, the NiTi clip produces less friction with NiTi wires. Research by Maria et al. demonstrates that self-ligating brackets exhibit significantly lower friction compared to traditional brackets, especially when combined with NiTi wires.
  • Convertible Design: Unlike the Damon bracket, the Pactive bracket can be converted to a regular bracket by removing the clip. The small hole on the clip allows for easy removal using a Weingart plier, allowing the bracket to accommodate regular elastic ties or metal archwires.
  • 80 Gauge Micro-Etched Mesh Base: Provides maximum bond strength with predictable and safe debonding.
  • Compatibility: Pactive is compatible with Innovation® from GAC and Empower® from American Orthodontics.

How to Open Pactive Self-Ligating Brackets

The Pactive smart clip has two mechanisms for opening the clip: from the face or from the base.

  1. From the Face: The Pactive clip has a small hole on its surface. Using the Ortho Plier Explorer “IOS 18-870”, apply gentle force towards the occlusal direction with a slight twist to open the clip.
  2. From the Base: Use the special Ortho Plier of “IOS 18-877” to apply occlusal pressure to the base of the clip. This technique is safer for the integrity of the clip, but may be harder if the patient has swollen gums or food accumulation.

Does Orthodontic Treatment Time Shorten with Self-Ligating Brackets?

Early clinical studies on treatment efficiency indicated an average reduction of 4 to 6 months in treatment time and four visits during active treatment. However, later prospective studies found no significant difference between the two bracket types. Another prospective trial comparing self-ligating brackets (SLB) and conventional metal brackets found no significant difference in severely crowded cases, but a slight advantage for self-ligating brackets in moderately crowded cases.
